51-64 | Capped at $50 for Business Groups (Normally $60) |
65+ | Capped at $50 for Business Groups (Normally $70) |
Enrollment Fee: Waived for Business Groups.
Family Coverage: Subscription fee waived for each child after the 5th family member. The fee for the spouse of a business group employee is capped at $50.
Flexible Billing: Charges paid by automatic credit card charge or bank draft. Billing processed monthly, quarterly, or annually according to patient preference with a 10% subscription discount for annual payment.
